Position Description

This team member talks with the resident, evaluates their problems and assists in finding solutions to assure that sufficient and appropriate social services are provided. They should be emotionally mature, objective and sensitive to people and their problems, dependable, work independently & maintain good working relationships with residents and co-workers.
Some duties include:
  • Making contact with the sponsor and/or family to report changes in health, current goals, discharge planning and to encourage them to participate in care planning
  • Assist staff to inform residents and/or sponsors about the resident's health status and health care choices and their ramifications
  • Discharge planning services
  • Arranging counseling services and behavior technician services
  • Meets the needs of residents and family members who are grieving and provides emotional support
  • Meets with care plan team on a regular basis
  • Participates in the MDS process by completing the social services portion of the assessments and ensuring that they are properly signed
Qualifications:
  • High school education or the equivalent
  • Experience in health care setting within a 2-year period
